CASE MANAGEMENT & PATIENT ADVOCACY SERVICES

Case Management – Our Case Management Department collaborates care coordination to meet our patient’s need and provide available community resources that are most cost effective or sometimes free. Services include: temporary housing, transportation, access to food services, specialist appointments, community resources, health fairs and more!

Patient Experience – There are sometimes in life when you need a helping hand. Our Patient Experience Coordinator helps the patient navigate through our services, onboards new patients, manage patient concerns, answer your questions, get you back on track if you haven’t seen a doctor in over 2 years, and more.

No. However, it does make it easier to connect you to our services if you are a patient as we will have all of your information.  We can connect you to back to your primary care provider.

REFERRALS

When you need to see a specialist, our Referral Department is here to help make the 1.2.3 step process is smooth and easy.

1. Make an Appointment – your FCHC primary care provider will need to see you before referring you to a medical specialist.

2. Prior Authorizations – Our Referral Coordinators will make sure that the specialist you’ve been referred to — or wish to see — has been approved by our FCHC primary care provider. Your referral can be picked up, faxed or mailed to the specialist.

3. Follow-up Appointment – Ensure you return for a follow-up visit with your FCHC primary care provider after your consult.

ARE YOU A PROVIDER REFERRING A PATIENT US?

We are happy to partner with you to care for your patient. Our referral department is here to help make it easy and efficient. Just fax our completed referral form or your own referral form, along with any necessary chart notes, to FAX: (708) 339-3583.

Our Referral Coordinators will call your patient, book his or her appointment and ensure you receive all related medical records in a timely manner. We can also coordinate appointments if you’re referring a patient to multiple FCHC providers.

Our goal is to make sure referring providers have all the information necessary to provide the best care possible for their patients. If you have any questions about your patients’ appointments, scheduled visits or chart notes, please feel free to call our referral department to have your questions answered.

IL Medicaid is year-round so you can apply at any time.  For Federal Marketplace, Open Enrollment starts Nov 1 – Dec 15.

Insurance renewal is called “Redetermination”.  The Redetermination period can either be every 6 months or once per year on your anniversary date.

In order to qualify, you must be a resident of the state of Illinois, in need of health care/insurance assistance, and whose financial situation would be characterized as low income or very low income. You must also be a U.S. national, citizen, or have satisfactory immigration status. To qualify for this benefit, you must:

  • Be over the age of 64; or
  • Be pregnant or have a child 18 or under; or
  • Be blind or disabled; or
  • Have a child, parent, or spouse in your household who is blind or disabled
